NOTE:  Submitting this form is ONLY a communication!  It is NOT to be confused with submitting your application, too!  We must have all pages of your application on file for each vendor, and each application must contain the vendor's original signature (not a copy).  Consignment vendors will operate on a single contract, signed just once.  Daily vendors will need to turn in either an application per month, with all desired dates marked and paid for, or an application per desired date.
